Background.

The Apgar score describes a newborn infant's condition at a specific moment after delivery, using five signs each graded 0, 1 or 2 for a total from 0 to 10. It is a clinical aid for clinicians and a shorthand for communication between them. The joint statement of the American Academy of Pediatrics and the American College of Obstetricians and Gynecologists is explicit about what it is not: the Apgar score alone cannot be considered evidence of, or a consequence of, asphyxia, does not predict individual neonatal mortality or neurologic outcome, and should not be used for that purpose. That sentence is the most important thing on this page and it is why the caveat sits beside the number rather than in a footnote.

The five signs are colour, heart rate, reflex irritability, muscle tone and respiratory effort — remembered by the backronym Appearance, Pulse, Grimace, Activity, Respiration, coined by Butterfield and Covey in 1962, nearly a decade after Virginia Apgar published the scale in 1953. Each sign is graded independently: 0 for the worst response, 1 for an intermediate one, 2 for the best. Because every sign can score 0, the floor of the total is genuinely 0, which is different from the Glasgow Coma Scale where the floor is 3. Carrying that intuition across is a common error.

This calculator prints the component summary alongside the total, because the total discards the information that matters. An infant scoring A2 P2 G2 A0 R0 — pink, well perfused, responsive, but floppy and not breathing — and an infant scoring A0 P2 G1 A1 R2 — dusky but crying vigorously — both total 6. Those are two very different babies with two very different immediate priorities, and "Apgar 6" says nothing about which one is in front of you.

Timing is part of the score's meaning. Assessment is made at one minute and again at five minutes in all infants, and when the score is below 7 the recommendation is to continue recording at five-minute intervals. The categories that this page reports — 7 to 10 reassuring, 4 to 6 moderately abnormal, 0 to 3 low — are defined by the AAP/ACOG statement for the five-minute score in term and late-preterm infants. They are not defined for the one-minute score, where a lower value in an infant who is transitioning normally is often unremarkable, and they are not defined for very preterm infants, in whom immaturity alone depresses tone and respiratory effort. The category output on this page carries that scope on its own label.

There is a small gap in the published heart-rate criterion worth knowing about. The grade is written as below 100 beats per minute for 1 point and over 100 for 2, which leaves a rate of exactly 100 unassigned; most bedside practice treats 100 or more as 2. This calculator does not resolve that for you, because it takes the graded response you assign at the cotside rather than a heart rate in beats per minute. The convention you apply at exactly 100 is yours, and it should be the one your unit uses.

The score is also not the resuscitation. Neonatal resuscitation is driven by respiratory effort, heart rate and tone assessed continuously, not by waiting for a minute to elapse and computing a sum. An infant who is apnoeic at fifteen seconds needs help at fifteen seconds. Apgar herself proposed the score as an evaluation, and the modern statement positions it as a description of the response to resuscitation as much as an indication for it — which means an Apgar score recorded during ongoing resuscitation should be documented as such, because the number reflects what is already being done.

Finally, the score cannot observe anything. Colour, tone and reflex response are judgements made by a person at the bedside, and inter-observer agreement on them is imperfect. A number entered from someone else's impression is an impression, not a measurement, and a page that turns five judgements into a confident integer should say so plainly rather than let the integer carry more weight than the observations behind it.

What is apgar score calculator?

The Apgar score is a five-item assessment of a newborn infant's condition, published by the anaesthesiologist Virginia Apgar in 1953 as a way of making the evaluation of the newborn systematic and comparable between observers. Each of five signs — colour, heart rate, reflex irritability, muscle tone and respiratory effort — is graded 0, 1 or 2, giving a total between 0 and 10.

It is assessed at one minute and again at five minutes after delivery in all infants, and continued at five-minute intervals when the score is below 7. Its enduring value is as a compact, standardised description of the infant's condition and of the response to any resuscitation being given.

The modern AAP/ACOG statement defines the five-minute score in term and late-preterm infants as reassuring at 7 to 10, moderately abnormal at 4 to 6, and low at 0 to 3, while stating equally clearly that the score alone is not evidence of asphyxia and does not predict outcome for an individual infant.

What it is not: a trigger for resuscitation, which is driven by continuous assessment rather than a one-minute sum; a diagnosis; a prognosis for a particular baby; a measure that means the same thing in a 26-week infant as in a term one; or a substitute for describing what the clinician actually observed.

How to use this calculator.

  1. Grade each sign independently from what you observe. The calculator takes your grade, not a measurement — it cannot see colour, tone or reflex response.
  2. Colour (Appearance): 2 entirely pink, 1 pink with blue extremities, 0 pale or blue.
  3. Heart rate (Pulse): 2 above 100 beats per minute, 1 below 100, 0 no heartbeat. At exactly 100 apply your unit's convention — the published wording leaves it unassigned and most practice treats 100 or more as 2.
  4. Reflex irritability (Grimace): 2 cries, coughs or sneezes on stimulation, 1 grimaces in response, 0 no reaction.
  5. Muscle tone (Activity): 2 active motion with flexed tone resisting extension, 1 some tone and flexion, 0 loose and floppy.
  6. Respiratory effort (Respiration): 2 crying vigorously, 1 slow and irregular, weak or gasping, 0 not breathing.
  7. Record the assessment time with the score. The AAP/ACOG categories are defined for the five-minute score; a one-minute score is a different observation.
  8. Hand over the component summary, not only the total, and note whether resuscitation was in progress when the score was taken.

The formula.

Apgar = A + P + G + A + R, each sign graded 0, 1 or 2 · total 0–10

The Apgar score is the sum of five signs, each graded 0, 1 or 2. Colour, heart rate, reflex irritability, muscle tone and respiratory effort contribute independently, so the total runs from 0 to 10. There is no weighting: a point of respiratory effort and a point of colour count the same, which is a known limitation of the instrument rather than a feature.

Rounding stage: there is none, and no fractional grade exists on the scale. Each sign takes a whole-number grade and the total is their exact integer sum. This calculator rejects a fractional grade rather than rounding it, because rounding 3.5 either way would move an infant between the low and moderately abnormal categories — a change of clinical label produced by an implementation detail rather than by an observation.

The floor is 0, not 3. Every sign's lowest grade is 0, so all five can be zero simultaneously. This differs from the Glasgow Coma Scale, whose axes all start at 1 and whose floor is therefore 3, and the two are often confused by people who work with both.

The grading criteria used here are the classical ones: colour 0 pale or blue, 1 pink with blue extremities, 2 entirely pink; heart rate 0 no heartbeat, 1 below 100 bpm, 2 above 100 bpm; reflex irritability 0 no reaction to stimulation, 1 grimacing in response, 2 cries, coughs or sneezes; muscle tone 0 loose and floppy, 1 some tone and flexion, 2 active motion with flexed tone resisting extension; respiratory effort 0 not breathing, 1 slow and irregular, weak or gasping, 2 crying vigorously.

Categories are applied to the exact integer total: 7 or more is reassuring, 4 to 6 is moderately abnormal, and 3 or below is low. Those cuts come from the AAP/ACOG statement and are defined there for the five-minute score in term and late-preterm infants. The calculator applies them to whatever total you enter, so the scope caveat travels with the category output rather than being assumed.

Worked through the example on this page: colour pink with blue extremities scores 1, a heart rate of 130 scores 2, a grimace to suction scores 1, some flexion without active resistance scores 1, and slow irregular breathing with a weak cry scores 1. The total is 1 + 2 + 1 + 1 + 1 = 6, the component summary is A1 P2 G1 A1 R1, and 6 falls in the moderately abnormal category — with the caveat that this was a one-minute score, and the categories are defined for the five-minute one.

A worked example.

Example

A term infant is assessed at one minute after an unremarkable vaginal delivery. The heart rate is 130 beats per minute. Breathing is present but slow and irregular, with a weak cry. There is some flexion of the limbs, but the baby does not actively resist extension. Gentle suction of a nostril produces a grimace rather than a cry or a cough. The trunk is pink; the hands and feet are blue. Grading each sign: colour is pink with blue extremities, which scores 1. Heart rate above 100 scores 2. A grimace in response to stimulation scores 1. Some tone and flexion scores 1. Slow, irregular, weak respiratory effort scores 1. The total is 1 + 2 + 1 + 1 + 1 = 6, and the component summary is A1 P2 G1 A1 R1. Six falls in the moderately abnormal category. The important qualification is that this is a one-minute score, and the AAP/ACOG categories are defined for the five-minute score in term and late-preterm infants. A one-minute 6 in an infant who is transitioning normally, with acrocyanosis and a settling respiratory pattern, is a common finding; the same 6 at five minutes is what triggers continued five-minutely scoring. The component summary is what makes the number actionable. Here the deficits are spread thinly across colour, tone, reflex and respiration with a reassuring heart rate. Compare an infant scoring A2 P2 G2 A0 R0 — also a total of 6, but pink, well perfused and responsive while floppy and not breathing. That is a completely different clinical situation reached by the same arithmetic, and only the summary distinguishes them.

Frequently asked questions.

Does a low Apgar score mean the baby was asphyxiated?
No. The AAP/ACOG joint statement is explicit that the Apgar score alone cannot be considered evidence of, or a consequence of, asphyxia. Diagnosing hypoxic-ischaemic injury requires far more information — cord gases, the clinical course, neurological examination, imaging and the exclusion of other causes. The statement notes that inappropriate use of the score has itself led to an erroneous definition of asphyxia. A low score describes a depressed infant at a moment; it does not name the reason.
Does the Apgar score predict how the child will develop?
Not for an individual. The same joint statement says the score does not predict individual neonatal mortality or neurologic outcome and should not be used for that purpose. Across large populations, low five-minute scores are associated with worse outcomes, which is why the score appears in epidemiological research — but a population association is not a prediction for one baby, and most infants with a low five-minute score do not go on to have cerebral palsy or developmental impairment.
Why is the floor 0 when the Glasgow Coma Scale floor is 3?
Because the two scales grade differently. Every Apgar sign has a lowest grade of 0, so all five can be zero and the total can be zero. Every GCS axis has a lowest grade of 1, so its floor is 1 + 1 + 1 = 3 and a GCS of 0 does not exist. Clinicians who use both regularly mix this up in documentation. An Apgar of 0 is a real, recordable value; a GCS of 0 is an error.
What score does a heart rate of exactly 100 get?
The published criterion says below 100 for 1 point and over 100 for 2, which leaves exactly 100 unassigned. Most bedside practice treats 100 or more as 2. This calculator does not decide for you: it takes the grade you assign rather than a rate in beats per minute, so the convention applied at exactly 100 is the one your unit uses. The gap is worth knowing about because it is one of the few places where two competent clinicians can score the same observation differently for a purely definitional reason.
Should the Apgar score be used to decide whether to start resuscitation?
No. Resuscitation is driven by continuous assessment of respiratory effort, heart rate and tone from the moment of birth, not by waiting for a minute to elapse and computing a sum. An apnoeic infant at fifteen seconds needs help at fifteen seconds. The score's role is to describe the infant's condition and the response to whatever is being done — which is why a score recorded during ongoing resuscitation should be documented as such, since it reflects the support already in place.
Do the categories apply to a preterm infant?
The AAP/ACOG categories — 7 to 10 reassuring, 4 to 6 moderately abnormal, 0 to 3 low — are stated for term and late-preterm infants. They are not defined for very preterm infants, in whom immaturity by itself lowers muscle tone and respiratory effort regardless of any hypoxic insult. Scoring a 27-week infant with the adult-of-the-newborn-world thresholds systematically labels immaturity as depression. Use the score to describe, and interpret it against the gestation, not against the table.
Why does this page show A-P-G-A-R components as well as the total?
Because a total of 6 can mean opposite things. A2 P2 G2 A0 R0 describes a pink, perfused, responsive infant who is floppy and not breathing. A0 P2 G1 A1 R2 describes a dusky infant who is crying vigorously. Both total 6, and both hand over as "Apgar 6" if only the sum is recorded. The summary is what survives a handover with the clinical picture intact, and it is what lets the next observer see which sign has changed at the next assessment.
How reliable is the score between different observers?
Less reliable than the crisp integer suggests. Three of the five signs — colour, tone and reflex irritability — are judgements rather than measurements, and agreement between observers on them is imperfect. The score is also affected by whether resuscitation is under way. None of that makes it useless; it makes it a structured description rather than a measurement, and it argues for recording what was observed alongside the number rather than letting the number stand alone.
