Background.
A Child-Pugh score calculator classifies cirrhosis severity from five variables: bilirubin, albumin, INR, ascites, and hepatic encephalopathy. Each variable receives 1, 2, or 3 points. The total score ranges from 5 to 15 and maps to class A, B, or C. Scores of 5 to 6 are class A, scores of 7 to 9 are class B, and scores of 10 to 15 are class C. The worked example uses bilirubin 2.8 mg/dL, albumin 2.9 g/dL, INR 1.8, mild or controlled ascites, and no encephalopathy. Those inputs score 2, 2, 2, 2, and 1 points, for a total of 9. A score of 9 is Child-Pugh class B.
This calculator has strong clinical search demand because Child-Pugh class appears in cirrhosis documentation, procedural risk discussions, hepatology notes, medication labeling, oncology eligibility criteria, and patient summaries. It is also a score that users often miscalculate because three inputs are laboratory thresholds while two inputs require clinical grading. A calculator should therefore separate the point assignment for each component and show the total. If the output simply says Class B without showing the points, the user cannot audit whether bilirubin or albumin was placed in the correct threshold band.
The clinical scope must be conservative. Child-Pugh scoring is not a diagnosis of cirrhosis and cannot be completed from labs alone. Ascites and encephalopathy are clinical findings. A user who does not know the ascites or encephalopathy category should not guess just to obtain a class. The calculator can provide category labels, but the category should come from clinical assessment, imaging, medication response, paracentesis history, mental status evaluation, and documentation by a qualified clinician. The page should also avoid using the result to make treatment promises. Class B or C may be relevant to prognosis or treatment risk, but the calculator does not choose therapy.
Implementation should make unit assumptions clear. The formula here uses bilirubin in mg/dL and albumin in g/dL, with INR for coagulation. Some regions document bilirubin in micromol/L or albumin in g/L. A production calculator can either offer unit conversion or state the expected units prominently. Silent unit mismatches are dangerous. For example, entering bilirubin 48 micromol/L into a mg/dL field would produce an impossible value and the wrong point assignment.
The best interface is a point table. The user enters lab values, chooses ascites and encephalopathy categories from controlled options, and the output shows each point row. The total score and class appear at the top, but the detailed table stays visible for audit. The calculator should include a medical limitation note, a source date, and links to primary literature and hepatology guideline sources. It should not replace clinician review, particularly when the score is used for medication decisions, surgery risk, or transplant discussions.
The page should also preserve the selected units and clinical category labels in any copied result so a chart reviewer can reconstruct how the class was assigned. That copied detail is important when the class is reused outside the calculator.
What is child-pugh score calculator?
The Child-Pugh score is a clinical scoring system used to classify the severity of chronic liver disease, especially cirrhosis. It assigns points for total bilirubin, serum albumin, INR or prothrombin-time-related coagulation status, ascites, and hepatic encephalopathy. Each component receives 1 to 3 points, with higher points indicating worse impairment or more severe complication status. The total score is then converted into class A, B, or C.
The score is partly objective and partly clinical. Bilirubin, albumin, and INR can be scored from laboratory values if the units are correct. Ascites and encephalopathy require clinical categorization. Ascites may be absent, mild or controlled, or moderate to severe. Encephalopathy may be absent, grade I-II, or grade III-IV. Because those categories require judgment, a calculator should not encourage users to invent them from vague symptoms.
Child-Pugh class is often used alongside other scores such as MELD or MELD-Na. The systems answer related but different questions. MELD-Na is a formula-driven score using laboratory values and sodium adjustment. Child-Pugh combines labs with decompensation findings. A hepatology workflow may need both, and the calculator should make its own assumptions transparent. That transparency is especially important when the class is later used in a medication label or procedural risk note.

The formula.
The Child-Pugh calculator assigns points to five components and then sums them. Bilirubin receives 1 point when it is below 2 mg/dL, 2 points when it is between 2 and 3 mg/dL inclusive, and 3 points when it is above 3 mg/dL. Albumin is scored in the opposite direction because lower albumin reflects worse synthetic function. Albumin above 3.5 g/dL receives 1 point, albumin from 2.8 to 3.5 g/dL receives 2 points, and albumin below 2.8 g/dL receives 3 points.
INR receives 1 point when it is below 1.7, 2 points from 1.7 to 2.3, and 3 points above 2.3. Some historical tables use prothrombin time prolongation instead of INR. This calculator uses INR because it is common in modern calculator interfaces and easier for users to enter directly. If a production implementation supports prothrombin time prolongation, it should make that a separate mode instead of mixing both measures in one field.
The ascites and encephalopathy inputs are categorical. No ascites receives 1 point, mild or controlled ascites receives 2 points, and moderate to severe or refractory ascites receives 3 points. No encephalopathy receives 1 point, grade I-II receives 2 points, and grade III-IV receives 3 points. The calculator uses dropdowns rather than free text so the output is consistent and testable.
After the five point values are assigned, the calculator adds them. The minimum possible total is 5 and the maximum is 15. Scores 5 and 6 are Child-Pugh class A. Scores 7, 8, and 9 are class B. Scores 10 through 15 are class C. In the worked example, the points are 2 for bilirubin, 2 for albumin, 2 for INR, 2 for ascites, and 1 for encephalopathy. The total is 9, which maps to class B.
A worked example.
The example uses bilirubin 2.8 mg/dL, albumin 2.9 g/dL, INR 1.8, mild or controlled ascites, and no hepatic encephalopathy. The bilirubin value is between 2 and 3 mg/dL, so it receives 2 points. The albumin value is between 2.8 and 3.5 g/dL, so it also receives 2 points. The INR value is between 1.7 and 2.3, so INR receives 2 points. The clinical categories are then scored. Mild or controlled ascites receives 2 points. No encephalopathy receives 1 point. The calculator adds the five point values: 2 plus 2 plus 2 plus 2 plus 1 equals 9. Child-Pugh class is assigned from the total score. A score of 9 falls in the 7 to 9 range, so the class is B. The output should show the point table as well as the class, because the class alone does not reveal which component drove the result.
